Help with passing dog

My dog is 17 and we think tonight maybe his last . I took him to the vet this morning and unfortunately couldn’t see the doc but saw the nurses , they said we could try one more Round of treats with antibiotics ( he has ongoing wound on paw since last August ) and revisit on Monday . However, the change in him is rapidly declining. I wanted to know if anyone knows of a place to call on weekends or at night if he were to pass in the palm beach / lake worth / Rivera area .
